A proposed organising tool · Not yet available
Walk into the next specialist visit with a one-page story packet.
For adults and family caregivers juggling rare or complex conditions across many providers — and tired of starting the story again.
The idea: an AI-assisted draft of a visit-specific one-pager and a short questions list, built from notes and documents you choose. You would check every detail, print or export, and share only if you want to.
This is an interest test, not a working medical service. No uploads, no payment, no product account today. Joining does not buy or reserve care.

A packet for one visit. Not another full records vault.
- A short timeline in your own words.
- Your top three points and questions for that appointment.
- Visible source labels so you can check the draft against your notes.
- A print-ready organiser you control.
Free passports and record binders may already meet your needs. This proposed tool tests whether drafting a fresh packet for each visit would save you work.
Not medical advice — organising tool only. Talk to your clinician. It would not diagnose, interpret labs, suggest treatments, replace a clinician or contact providers for you. AI can make mistakes; every draft would need your review.

Fictional example — not a real patient or a clinical recommendation. Adapted from Lumen's fictional neurology handoff sample. Bracketed fields are intentionally empty, not missing facts filled in by AI.
Neurology follow-up · Patient-authored draft
- Visit reason
- [Your words about this appointment]
- What I want this team to know
- “I bring printed history because portals alone have not stopped the re-explaining.” — fictional example note F1
- Timeline highlights
- [Date] · [Event in your words] · [Source document ID]
- Current list to check
- [Your medication / device list, unchanged from your source]
- Questions I want to ask
- Which records from prior centres are still missing on your side?
What observations should we track between visits? - Sources and review
- F1 = fictional personal note; all other fields await user input. Review every detail before sharing.
Drafted by Lumen, an AI. Organising aid, not diagnosis or treatment advice.
Would this be worth it for your next visit?
Proposed price: $9 / month
Variant A: visit packets during your subscribed month. The subscription and product do not exist yet. No charge today.
Considering a one-off option instead?
We are also testing a proposed $12 per packet option (Variant B), not offered for purchase. A later research-only variant is a $0 vault plus $29 for a first packet (Variant C); it is not the first launch test. All current waitlist sign-ups below relate to the $9/month proposal.
Waitlist privacy
For this waitlist and one launch update, we collect email and consent — no health data on the waitlist. We also attach a broad source tag (not a condition or personal identifier). Do not enter health information in your email address. The host and form provider may process technical metadata such as IP address and browser information.
Planned processing: Cloudflare hosts the page; Formspree processes the email form, including in the US. We do not sell waitlist information or add you to unrelated marketing. Waitlist records are scheduled for removal within 30 days of submission, including notification copies under our control. If we kill the idea, we will also delete remaining records within 30 days of that decision (without extending the submission-based deadline). Providers may retain their own security logs or backups under their policies.
For replies, removal or withdrawal, contact lumenhealth@mail.grokbot.com. Your interest in a health-related tool can itself be sensitive: joining is optional. No health documents will be accepted during this test.
See Formspree privacy and Cloudflare privacy. If enabled at launch, Cloudflare Web Analytics measures aggregate visits without cookies or cross-site tracking; blocked analytics may undercount visits.
